Lead Transplant Coordinator

Lead Transplant Coordinators provide clinical supervision and use a multidisciplinary approach to coordinate unit/area clinical operations with other departmental disciplines, coordinate transplant program activities, including QAPI Project and Unit-level PI planning and oversight, provide effective and efficient patient care, perform both a clinical and administrative role (50/50), and assume the responsibilities of the Transplant Clinical Manager in their absence. 

A caregiver in this position works 4-10s, 7:00am-5:30pm, with the opportunity to work from home 1-2 days a week as department needs allow. 

A caregiver who excels in this role will:  

  • Actively participate in and facilitate communication and collaboration among and between organ division caregivers, the multidisciplinary team, and other departments regarding issues related to transplant patient care, ensuring continuity of communication of patient care information between members of the multidisciplinary team. 

  • Participate in and support QAPI projects, outcomes studies, research initiatives and PI activities as assigned. 

  • Serve as part of departmental and Transplant Center committees and as a resource to transplant coordinators and transplant patients to triage issues as they arise and to ensure rapid resolution. 

  • Collaborate with the Clinical Nurse Manager to prepare work assignments within designated department to assure adequate coverage for Transplant Center clinical operations. 

  • Coordinate transplant program activities to maintain the continuity of patient care throughout the transplant patient clinical path. 

  • Participate in and supervise the daily delivery of clinical and operational care at the departmental level within the assigned organ division. 

  • Review operational patient care issues with the Transplant Clinical Manager. 

  • Cascade patient complications/adverse events to the Clinical Nurse Manager so that these can be triaged appropriately. 

  • Coordinate with the Transplant Clinical Nurse Manager to provide ongoing clinical evaluations of nurses and other direct/indirect patient care providers on assigned floor(s) and alert the Transplant Clinical Nurse Manager of clinical or other performance issues. 

  • Collaborate with the Clinical Nurse Manager to monitor caregiver attendance and adherence to established CC and Transplant Center policies, procedures and guidelines. 

  • Assist in orienting/on-boarding new caregivers, continuing education and in-service educational programs. 

  • Maintain Transplant Center and nursing competencies. 

Minimum qualifications for the ideal future caregiver include:  

  • Graduate from an accredited school of nursing.  

  • Bachelor of science in nursing (BSN). 

  • Current state licensure as a Registered Nurse (RN).  

  • Basic Life Support (BLS) certification through the American Heart Association (AHA). 

  • Four years of transplant experience with organ procurement or transplantation including disease related experience.  

  • Current demonstrated clinical competence.  

Preferred qualifications for the ideal future caregiver include:  

  • Previous transplant coordinator or related experience
