Patient Intake Coordinator (Remote)

The Patient Intake Coordinator for Medrina360 Care at Home is responsible for managing the patient intake process for a specialized home-based physician services program Medrina360 Care at Home. This role involves collaborating with healthcare providers, patients, families, and administrative staff to ensure that all admissions are processed smoothly and that patients receive timely, coordinated care. The ideal candidate will be detail-oriented, compassionate, and adept at managing multiple tasks while maintaining a high level of customer service. This is a full-time, remote position, residing in Chicagoland area is a requirement. This role requires occasional evening or weekend work to meet patient and organizational needs. This position offers $75,000 salary with benefits inclusive of 15 days of vacation, sick days, 401k Plan with company match, immediate health/dental/vision insurance and more. Key Responsibilities:
  • Patient Intake Management:
    • Serve as the primary point of contact for new patient inquiries and referrals.
    • Collect, verify, and document patient information, insurance details, and medical histories.
    • Ensure all intake documentation complies with legal, regulatory, and organizational standards.
  • Coordination & Communication:
    • Collaborate with physicians, nurses, case managers, and other healthcare professionals to facilitate patient admissions.
    • Coordinate pre-admission assessments and schedule initial consultations.
    • Maintain clear and timely communication with patients and families regarding admission status, care plans, and next steps.
  • Administrative Duties:
    • Manage patient records and update electronic health records (EHR) accurately.
    • Generate and track referral reports, ensuring timely follow-up.
    • Assist with billing inquiries and documentation related to patient intake.
  • Quality Assurance & Compliance:
    • Monitor intake processes to ensure consistency and adherence to organizational policies and regulatory requirements.
    • Identify and recommend improvements to enhance workflow efficiency and patient satisfaction.
  • Customer Service & Support:
    • Provide empathetic support to patients and their families, answering questions and addressing concerns throughout the admission process.
    • Serve as a liaison between patients and the clinical team to ensure a seamless transition to home-based care.
  • Skills & Competencies:
    • Excellent communication and interpersonal skills.
    • Strong organizational skills with attention to detail and ability to manage multiple tasks simultaneously.
    • Proficiency in electronic health record (EHR) systems and basic computer applications.
    • Ability to work independently and as part of a multidisciplinary team.
    • Strong problem-solving abilities and a customer-focused mindset.
Requirements:
  • Job is entirely remote. Candidates must reside in Chicagoland area.
  • Occasional evening or weekend work to meet patient and organizational needs.
  • 5 years experience in a healthcare setting, in a patient intake, or admissions, or case management role.
  • Associate or bachelors degree in healthcare administration, Nursing; or 10 years of experience in relevant healthcare settings: patient intake, nursing required.
  • Proficiency in electronic health record (EHR) systems and basic computer applications.
